Talk

Adrian D'Costa
ABOUT

Adrian D’Costa is a physiotherapist, clinic founder and running performance specialist based in London. He is the founder of The Running Room London, a physiotherapy and performance clinic built to help runners move better, train smarter and stay healthy for longer.

Adrian’s work sits at the intersection of physiotherapy, endurance performance, strength training and community. His clinical interests include running related injuries, hip and groin pain, knee pain, ACL rehabilitation and tendon related injuries. He combines detailed clinical assessment, strength testing, gait and running analysis, rehabilitation programming and practical training advice to help runners understand their bodies and return to training with clarity and confidence.

Over the course of his career, Adrian has developed a particular expertise in supporting endurance and ultra endurance runners. He has worked with athletes preparing for and completing demanding running challenges, giving him a deep understanding of the physical, psychological and logistical demands that come with long distance performance. This experience has helped him become one of the leading physiotherapists in the running and endurance space, with a specialist ability to manage injury risk, training load, recovery, strength development and return to running for athletes taking on extreme challenges.

Before moving to London, Adrian spent several years developing his clinical skills at iMove Physiotherapy in Sydney, working within a high performing sports physiotherapy environment. Since relocating to the UK, he has continued to grow his clinical practice through R&D Physio and The Running Room, bringing together technical expertise, performance focused rehabilitation and a strong commitment to building better systems of care for runners.

Adrian’s background as an athlete has also shaped the way he works. Having played a range of competitive sports including football, rugby, cricket and basketball, he understands the demands of training, competing and returning from injury. This allows him to connect with both recreational and competitive athletes, helping them balance ambition with long term resilience.

As the founder of The Running Room London, Adrian has helped build a clinic that supports runners of all levels, from first time runners to experienced marathoners and ultra endurance athletes. Through physiotherapy, running assessments, gait analysis, performance testing, strength programming and return to run rehabilitation, Adrian and his team provide a complete support system for runners looking to recover, perform and stay consistent.

Beyond the treatment room, Adrian is passionate about making running more inclusive and accessible. He is actively involved in community initiatives that support first time runners, particularly those from ethnically diverse backgrounds and people of colour. He believes running spaces should feel welcoming, representative and empowering.

Education is also central to Adrian’s work. Through his social media platform, @thefreshmanphysio, he mentors and educates a physiotherapy audience of more than 20,000 people, sharing insights on clinical reasoning, rehabilitation, communication and career development. He is particularly passionate about supporting early career physiotherapists and helping them build confidence in their clinical decision making.

Adrian’s work spans clinical practice, business development, team leadership, education, community building and endurance performance. Across all of these areas, his mission remains consistent: to help people move well, build strength, understand their bodies and feel capable in the activities that matter most to them.
